On 9 April 1944, while assembling for a combat mission to Tutow Airfield, Germany, the Aircraft was involved in a mid-air collision and crashed 2 Miles SW of Fulsham, Norfolk, England.

Poor wheather conditions with considerable middle cloud resulted in confusion in assembling and caused this collision. Lt. Morefield and Lt. Kaufman were the only men who parachuted from the Aircraft. The remaining crew members remained in the aircraft and were killed in the crash.
